Position: Manager of Case Management

The Manager of Case Management has overall responsibility for the day to day leadership of the department and promotes effective and efficient utilization of resources to support the patient needs throughout the continuum of care, identify outliers, and develop action plans for improvement.  This job   partners with the Director in translating and operationalizing strategic initiatives, managing, motivating and directing the front line staff and leadership in the support of quality patient care, safety, financial improvements, and optimal patient flow, thereby promoting an integrated and seamless patient-centered experience.  The manager serves as the subject matter expert and resource for all departments related to case management activities; promotes continuity of care and cost effectiveness through the integration and functions of case management; and supports and implements an integrated case management model throughout the Roper St Francis Healthcare delivery system.  The manager develops and maintains professional networks and individual relationships with hospitals, physicians, and other internal and external stakeholders in order to promote continuity and quality of care. 

Minimum Qualifications:

Education:  BSN required. MHA, MBA, or MSN preferred.  Candidates with national certification in case management fields will be given preference.

Experience:  Five years clinical experience and a minimum of two years supervisory or managerial experience required. Experience with case management, discharge planning and utilization management in a healthcare system.  Demonstrated knowledge of the healthcare industry, current legislation, regulations and reimbursement, as well as a proven track record in management of length of stay and cost per case and effective physician relations.

Licensure/Certification:  Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.

Primary Source Verification (if applicable): RN- Nursing- SC Labor, Licensing and Regulation (LLR): or compact RN state licensing board.

Knowledge/Skills:  This role requires experience and leadership in managing the case management process for government and commercial payers and must have working knowledge of federal and commercial regulations as it relates to care coordination. The successful candidate will demonstrate a strong track record of building and leading teams. He/she will balance vision with a hands-on approach and will be comfortable driving results independently, as well as with and through others. Ability to navigate within ambiguity, a service orientation, and a high level of humility are all requirements for successful assimilation into our highly collaborative culture. Demonstrated ability to engage and manage across departments within the hospital organization and throughout the healthcare community.  Must have demonstrated ability to develop and implement policies and procedures and evidence-based practices and standardized work flows across the organization.  Proficient in communicating clearly and effectively with multiple constituents. Proficient in computer usage, especially database and spreadsheet analysis.  Knowledge of organizational policies and procedures. Familiarity with risk stratification models.

Other:   Subject matter expert on care transitions, discharge planning, continuity of care, utilization review and quality improvement concepts and practice.  Must have superb written and verbal communication skills and demonstrated commitment to collaborative practice and continuous quality improvement. Fluent in system-wide PC applications Excel, Word and PowerPoint.  Working knowledge of clinical systems where applicable which may include WINPFS, STAR, CareManager, Cerner, etc.
